Air St. Lukes Membership Application

In cooperation with Air St Luke's, the Idaho ATV Association strongly recommends that our members take advantage of Air St Luke's membership offer of $35.00 per year, which includes membership benefits for yourself, your spouse and any dependent children under the age of 23 that are claimed on your income tax. In order to receive the $35.00 rate please send them a copy of your membership card. If you are submitting a hard copy at the top of the application please write "Idaho ATV Association"
